STEP 3:

Bring the necessary documents to your appointment

Checklist:

  • ALL active medical insurance cards
  • Photo ID
  • Physical medications / pill bottles
  • Printed pre-appointment forms (only if you chose not to fill online form)

Come to your first appointment!
