Instructions for CDL exam:

1. Call office to Schedule Appointment, (Plan for 1 hour of time for the entire process)

2. Complete CDL forms below. Please use our preprinted forms to speed up the Appointment

3. Be prepared! Your Medical provider MUST know all medications you are taking, as well as medical history

4. Day of the appointment: Take your medications per your routine. Bring your prescription glasses, Don’t smoke the morning off, and it is recommended to not drink alcohol the night before. 

5. If forms completed, arrive to the office at least 15 minutes prior to schedule time to be sure all forms are completed properly. If no forms, Please come at least 30 minutes prior 

Payment for the exam is taken prior to meeting with medical provider.
